Welcome to the SID Tutorial

The Services Infomation Database (SID) will help you to locate the services that you need for yourself or a loved one. It is designed with simplicity and ease of use in mind with an overriding goal of helping you connect with the service provider that is right for you. This demo will show you its basic functionality.

	Fill in your zip code Put your zip code in the blank following the line "Please Enter Your Zip Code:". This narrows the search down to service providers in your area.
	Select the type of service provider Select the type of service provider you are looking for from the pull-down menu. You will have choices like Speech Therapist, Occupational Therapist, Music Therapist, etc.
	Select the contracted agency Select the agency that you are getting services from. For example, you could choose DDD (the Division of Developmental Disabilities), Prvt Ins (Private Insurance), etc. from the pull-down menu.
	Agree to the terms and conditions Check the box indicating that you agree to the terms and conditions.
